Release of Liability *

I understand that participation in Iconic Sport Performance (ISP) activities is voluntary, and I acknowledge the inherent risks involved in participating in physically demanding activities, including but not limited to strength training, agility drills, and sport-specific exercises (hereinafter referred to as the "Activity"). I acknowledge, understand, appreciate and agree that my participation may result in possible exposure to and illness from infectious diseases, including, but not limited to, MRSA, Influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation and exposure. I hereby give my approval of and consent to my child(ren)'s participation in the Activity. I assume all risks and hazards incidental to the Activity and to transportation to and from the Activity. I hereby release, acquit, covenant not to sue, and forever discharge, and agree to indemnify and save harmless Iconic Sport Performance its owners, officers, administrators, employees, agents, volunteers, sponsors, advertisers, coaches and supervisors, and the owners or lessors of any facilities within which the Activity is conducted, their agents and employees, and all other persons providing facilities or assisting in the conduct of the Activity and in the transportation of participants to and from the Activity, of and from any and all actions, causes of action, claims, or demands, of whatever name or nature arising out of injuries to or death of the above named child(ren) as a result of the Activity and the transportation of the above named child(ren) thereto and there from.

Photo & Video Release

I certify that my signature being affixed below on this consent form gives permission to Iconic Sport Performance, LLC the full right to use my photograph(s), videotaped image and sound byte in its marketing, public relations, promotional or instructional efforts. I willingly agreed to have my photograph(s), videotaped image, sound byte taken knowing that it could be used in various publications. I hereby waive any right that I may have to inspect and/or approve the finished product or the advertising copy that may be used in connection therewith, or the use to which may be applied.

Medical Release *

I authorize the designated employees of Iconic Sport Performance, LLC to authorize on my behalf all appropriate medical treatment that may be required in the event of illness or injury to me resulting in any manner from participating in this training program. This authority is intended to cover any illness or injury sustained while participating in any activity supervised by a personal trainer at Iconic Sport Performance, LLC.

Return & Refund Policy *

Refunds are not provided, except in extenuating circumstances such as family emergencies or injuries, which will be reviewed on a case-by-case basis. All refund requests must be submitted in writing with supporting documentation. Approved refunds will be issued as account credits unless otherwise determined by management.

Trial Make-Up Policy *

As part of our commitment to providing you with a top-tier experience, we offer a complimentary "First Free Workout" for all new clients. Please note the following conditions: If you choose not to attend your scheduled session or fail to reschedule within 24 hours prior to the original session time, you will forfeit your complimentary workout and will be charged the current drop-in rate for the respective class. There is no charge for failure to attend a session, but the "First Free Workout" will be considered forfeited under these circumstances

Precautionary Coronavirus Liability Release Form

I understand the above symptoms and affirm that I, as well as all household members, do not currently have, nor have experienced the symptoms listed above within the last 14 days

I affirm that I, as well as all household members, have not been diagnosed with COVID- 19 within the last 30 days.

I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 within the last 30 days.

I affirm that I, as well as all household members, have not traveled outside of the country, or to any city outside of our own that is or has been considered a “hot spot” for COVID-19 infections within the last 30 days.

I understand that this business cannot be held liable for any exposure to the virus or any other contagion caused by misinformation on this form or the health history provided by each client.

By signing below, I agree to each above statement and release the trainer and business from any and all liability for the unintentional exposure or harm due to COVID-19.

All employees of this facility agree that they abide by these same standards and affirm the same. We also affirm that we have improved and expanded our sanitation protocols to thoroughly fight the spread of COVID-19 and other communicable conditions.
